Medical (gas Vacuum System Verifier)

Medical Gas and Vacuum Systems Verifier License: Louisiana Medical Gas and Vacuum Systems Verification law (LA R.S. 37:1361 et seq.) requires that persons engaged in the work or business of testing and verifying medical gas and vacuum pipeline installations and systems shall possess a license of renewal thereof issued by the State Plumbing Board of Louisiana. Your license will expire on December 31 of the license year.

Must complete 32 hours of Medical Gas Installation training that meets the criteria prescribed by the Board and NFPA99C Gas and Vacuum Systems.

Plumber, Journeyman

Must perform 8,000 hours of manual labor of plumbing under direct supervision of licensed journeyman plumber.

Successfully pass required examination, and pay appropriate fees.

Sprinkler System Installer

Employees must hold a current certification as a NICET LEVEL III or IV in Sprinkler Layout http://www.nicet.org/. An inspector must pass a competency exam administered by the State Fire Marshal’s Office.

Applicant must attach two color photographs that are one inch by one inch and shows the whole face.  Failure to do so will result in a delay of the process. We cannot process the application without the photos, dates, including the date of hire, and must have original signatures!

Must submit a photocopy of the applicant’s social security card and driver’s license.  This will enable our office to verify required information in case of illegible handwriting.

Certificate Holder must attach a copy of current NICET Card and sign and notarize the attached affidavit, for an initial application and or renewal.

No cash or personal checks are accepted from firms or employees! Company check only (Certified Funds for firms), or money order should be made payable to the Office of the State Fire Marshall.
